Honestly, no game up until Reach : clashed so blatantly with the rest of the canon. It wouldn't be such an : issue if it was something small like, say, Sgt. Stacker and Chips Dubbo : somehow escaping Halo. Which isn't even an issue since Stacker and Dubbo are, in-game, treated as reusable stock characters and aren't actually intended to literally be the same person in all their appearances. Only Halopedia treats it otherwise. All logic and extrapolation from standard practices says they aren't literally specific characters in-game. In fact, it might be good to note that according to HCE, nobody except for MC escapes Halo. I actually wish they had kept it that way, since I've always felt that they didn't really need the Johnson of H2 and H3 to be THAT Johnson guy. It's kind of a slap in the face to the brilliant atmosphere of the first game.
