Now that's a good question... I wouldn't want to commit to saying that it has never happened, but it doesn't seem to be part of any current doctrines.

Though its a lot harder to get good information and images from certain forces, such as the old Soviet Navy or the delightfully named "People's Liberation Army Navy".
Certainly the Soviets leaned more towards numbers rather than names for a ships class (most class names for Soviet ships are derived from NATO rather than the Soviets. The Russians never referred to the MiG-21 as the "Fishbed", though they were quite flattered by the MiG-29 "Fulcrum" and adopted that as a nickname).
