It's kind of like I told you. In the old days the publisher would retype everything, even if it was given to them in digital form.

Nowadays they probably just mess it up when they are reformatting. Selecting, cutting and pasting is quite prone to errors. Then the same no-mind 'fixes it up'. Gah!
