I think the Index is indeed a physical object. At the end of The Library, the Chief picks it up, and 343GS removes it with his energy arc. It's bigger than he is, so I'm guessing he has a small built-in teleporter which he used to move the Index to his "closet" or something. At the beginning of Two Betrayals, the Chief gets the Index back and plugs it into the console, where it remains until the end of the game. The Index Cortana can be seen holding is a holographic representation of it Cortana generated, perhaps to indicate what she was talking about. Also, I don't think Cortana is directly affecting 343GS's thrusters, I think he lost control over them due to his surprise, anger, or devoting all of his attention to fighting Cortana inside the Core.
