I've actually done this, but haven't had the time to package them up. Here they are without any tags or artwork. The spaced-joined file is a CUE file next to a single M4A with an extra 1.5 seconds between each track. The script uses shntool, alac, sox, and normalize (all from MacPorts), as well as afconvert. Easy peasy :-)
