That's why the best mods have excellent balancing. Mother 1 has an easy patch that makes the game fair, (the original difficulty is ludicrous, especially as it's random turn-based battles) Eternal is arguably no harder than Infinity on Total Carnage whereas Evil is impossible on major damage and Rubicon simply was never tested on Total Carnage. (Rubicon X anyway) Brutal Doom makes that game harder on top of being bloodier but you know that's the idea going in. It even has a Power Fantasy difficulty in the latest build that's basically SPV3's Noble difficulty if it were good: it's the same as Total Carnage and Mythic (Nightmare) but enemies are piss easy to beat. It's intended to make harder vanilla WADs like Final Doom playable. I've tried Plutonia on an older Brutal Doom build on Major Damage (Hurt Me Plenty) and it pulls no punches on later maps. System Shock 1 mods would also be very tricky to do right as it has multiple difficulty settings for combat, hotwire lockpicking puzzles, cyberspace hacking and even the story mode: 0 is Doom level, 1 has basic and brief audio logs, 2 is the main mode and 3 adds a Pathways into Darkness style 7 hour time limit.
