This would mean that any time you have the reticle on a target, the shot will hit (at least if the target isn't moving). Which it doesn't, as is pretty obvious at high ranges.

I'm betting it just nudges the bullet angle a little in the direction of a defined "center" position on a target, rather than directly placing the shot on the target. It's entirely possible that aiming a little high will drastically increase the tendency of the adjustment causing a shot to land on the head area, though.
Still, it's hard to expect even grunts to be completely easy 1-shot kills; Halo 1 doesn't help the player as much as its successors do. It's not like you get H2 or Reach levels of magnetism and a 3-shot headshot burst.

Possibly, though it's hard to tell; sweep targeting is a fairly natural way to pull off a shot and would probably be effective anyway, as the player only needs to worry about the angle of the sweep and the timing of the shot. They don't also have to think about feathering the aiming stick around.
