I selected the level, got the skull, saved and quit. I did it in one sitting, which was extremely long and arduous thanks to the IWHBYD skull. I'm still not entirely certain how exactly to spawn it, but the point I was at, I got a checkpoint just before jumping into the balcony. Jump in, no skull, wait a few seconds, and the skull spawned, almost as if it were on a timer. Dying on the waves of elites took me back out, where I could spawn the skull again.
