Photoshop.
New Layer.
Fill with green.
Set transfer mode to "color".
New layer.
Filter->Noise->Add Noise. Fiddle to your liking.
Set transfer mode to "overlay". Maybe fiddle with transparency.
New layer.
Fill with black.
Pick horizontal-row tool.
Click-and-drag to the top of the screen. Delete, nudge down two rows, delete, etc etc etc.
(Alternately, delete two rows, skip two, delete two, etc).
Play with transparency to your liking. Possibly also try transfer mode of "overlay".
That should do it. You might also want to play with the brightness/contrast of the original image.
