REPORT FOLLOWS On 03/10/2612, 0013, CN Lewis, David entered airlock 12-C. PFC Worth, John was working on repairs to ship following accident on 03/05/2612. (SEE LOG) A faulty sensor caused the inner door of the airlock chamber to be left open approximately 8.4 cm. Upon opening, there was massive depressurization of the compartment. Lockdown activated successfully. Inner airlock door did not attempt to reclose. PFC Worth was declared dead upon EVA-equipped team's entry. Event is still under investigation. No cause has yet been found. Audio, video and telemetry data is corrupt. *************

I will also re-catalog all weapons and ammunition aboard the ship in accordance to Rolenc Protocol VI.
