My list isn't complete (I don't have the Halo Encyclopedia yet), but I think the only nations that have been mentioned by name are: United Republic of America - made up of Canada, Mexico, and "the political remnants of the United States." Cuba - home of the Centennial Orbital Elevator Deutschland - The German word for Germany. A successor (or neighbor?) of the 2100s-era Unified German Republic. East African Protectorate - Either a combination of the old colonial territories of British and German East Africa (Kenya and Tanzania) or a descendant of the now-theoretical East African Community (Kenya, Tanzania, Uganda, Rwanda, and Burundi). United Korea - A unification of North and South Korea. Also known as Hanguk (the Korean word for Korea). Australia - home of one of the UNSC's main headquarters. Switzerland, Chile, and Mongolia - all mentioned in ILB. Additionally, the names of some off-world settlements suggest that South Africa, Japan, and the Philippines are still around, or at least survived long enough to colonize. Also, the company SinoViet implies an association between China and Vietnam. Based on that, here's a map of what's accounted for:
