It sounds to me like Cassandra is that one Spartan who was too badly injured to continue active duty. (pg.240, Fall of Reach, I think) It pretty much lines up, as that Spartan would know about MC's serial number, would be close enough to him that she would know what had happened on Halo, and would have a connection with Johnson. Not to mention this would make sense if Cassandra was at a recovery station.
