Most browsers cache these things forever. This can lead to seeing old favicons after they've changed, or not seeing any custom favicon after viewing a site that for a long time didn't have one.
There are ways to force Safari to remove the cached favictons:

There are also utilities that force Safari to NEVER cache favicons. So you'll always be looking at the right one, even if it costs you a little more bandwidth.
