I think it's pretty clear they didn't have Reach planned when they were making Halo 2, and they said they didn't even decide on the subject matter of the game until they were almost done with Halo 3, so that should give a general time frame. The planning for ODST, well, I'm not sure what to say there. As for Halo 3, I'm not aware of any notable continuity issues, in fact most of the issues it does have seem to have to do with not being able to make Halo 2 according to plan. If you want to criticize them for not making a fuller, longer-reaching plan once they knew it was a hit, I won't argue with you (unless I see some kind of contradictory proof).
