I'd replace that extension (as it may be corruted), : and see if that works. Sounds like a hardware problem to me. If he's already replaced the AC adapter once, it's probably something in the powerbook itself. Or he got two bad AC adapters. I've got a spare 45W adapter that I _know_ works from a old powerbook if he wants to try it...
