I would suggest the M1 scenario Marathon Revisited - the author didn't bother to change the terminal text, and he was inappropriately demeaning to women, using pornographic images and "Boobs" instead of Bobs (I'm sure that Shebob would have something to say about that), but the levels are exceptionally well-constructed, most of the sprites have been modified and some new monsters have been added. It's a lot of fun to play.

I think that this thread is getting too long, so I'm going to start a new thread on unsung scenarios.
