To get to Flashback: don't pickup the fusion pistol and finish the level.

To get to Entryway: stand on one of the corners of the Mirata in outer space.

To get to A Good Way To Die: when you get to the exit term on Neither High nor Low, don't read it. Go back and punch the switch you couldn't flip (which you may now, unless you stand too far from the terminal.) Now go back to the 3 pillars room: the door to the secret teleporter must be open.
