I had a similar problem and fixed it by setting my output frequency in Utilities\Audio MIDI Setup to 44.1 KHz, a value that I recognized. 96 KHz is kinda insane.
But, when I sought help for this, the very first thing treellama asked me was 'Are you running Tiger?' and I certainly am, so you might have a different problem altogether, or at the very least a different solution.
