As for the extermination question, I did extensive research concerning this very topic back when we discussed the M1 level "Try again". That is the only level of M1 that contains Juggernauts. It is also an extermination level. The rule for an M1 extermination level is as follows: You are allowed to leave 8 enemies alive except that you MUST defeat all Juggernauts. That is, if you have defeated all enemies except for even ONE Jugg, the level will not let you exit. On the other hand, if you have defeated all Juggs, then you are permitted to leave no more than 8 other enemies alive in order to leave the level. So my guess is that either you haven't defeated both Juggernauts, or if you have, then you have more than 8 enemies still alive somewhere on the level.
