I guess you haven't seen the movie in a while? Vasquez fires two grenades during the retreat from Operations, then Ripley fires four in the Queen's lair. The fully working M41A prop had separate triggers for the rifle part and the launcher part - specifically the existing triggers of the Thompson submachine gun and Remington shotgun the prop was constructed from. It was technically possible to fire both at once, though it would be extremely awkward and the launcher/shotgun would have to be manually cycled after the first round. In any case, spamming grenades and automatic fire at the same time is not particularly useful outside the specific context of games like Marathon.
