Rampancy goes fishing

Narcogen and Blackstar do level 5 - Couch Fishing. Fun stuff.

Lots of deaths including a team kill.
"Do you want me to team kill you?" :)
Unfortunately, the whole level is a suicide trap if you fail to flip all the switches and get to the end without keeping that last door open. It's activated by an on/off switch so flipping it again while the door is open will keep it open. Check the Vidmaster page for this tip.

In the original game you would have got off the level since you did flip the single switch necessary to successfully complete the level. In M1A1 you need to flip all three, as per Leela's instructions.
You missed the secret ammo cache near the start. The Spoiler Guide does actually mention this but NOT under the Secrets section. I have no idea why. It is a secret.
No bonus material from Defend THIS!. :(
Anyways, fun video of a hard and complicated Jason Jones level. Keep them coming. :)
