Hats Off to Eight Nineteen is Double Aught taking their hats off (in reverence or respect) to Hamish Sinclair. You'll notice that H and S are the eighth and nineteenth letters of the alphabet.

This is the generally accepted reason for the name, but I'm sure someone more versed in the topic than I am will have to tell you more about the rest of your theory.

Also, as for 'when did we have this dream,' I'm pretty sure that the clause 'as if from an old dream' is just a way of making the mode of remembering more stylized. It's just poetry tucked inside of prose, nothing more. To take it in that he literally had a dream about the decompression of his shuttle might not be a wholly accurate move.
