Steve, I had a vid film of this level so I was able to play it back and review the level. After thinking about the problem you are having, I also seem to recall having some problems with these two chip slots. I wish I could be more precise, but it's been a long time since I played the level. However, in my film, I first read the terminal in between the two slots, and then inserted the first chip in the slot on the right. I have no idea if it's necessary to read the terminal first, but that is what I did. After getting the second chip, I inserted it into the left slot, read the terminal again and then completed the level.

: The level is Aquae Perterbae - there are a pair of chip slots on either side
: of a terminal - the terminal is due East of the entry point and exit drop
: chute, near the center of the map.
