Actually, the Vash's gun is based off a real gun. The barrel is placed down the midline of the gun, so that when you fire the barrel is centered in front of your hand, rather than above it, so you have reduced recoil. It's a mix of a modifed .45 long Colt (extended barrel, uses higher power .45 bullets) and some kind of newer revolver with the barrel placed lower (can't remember the name). If I can find a picture of the real gun it'll explain what i mean better.
