I would agree with you on the lever-actioned idea. If you aren't familiar with the concept of a level-actioned weapon, take a look at a Sharp's rifle.

How this would apply to a double barreled shotgun, i don't know. Furthurmore, I don't see how one could fire that thing and not tear off a good section of your forearm. I like to let the idea of futuristic shock-absorbing materials and a chamber with a built-in teleporter fill the gaps for me.
