Perhaps there is only the one Halo that had Flood samples. I think this is actually very likely. As for finding Halo, in the book Halo: The Fall of Reach (FoR), Cortana interprets some symbols found in a rock (from a 50,000 year old meteor that impacted on Reach). The symbols are some kind of star navigation language and Cortana (without telling anyone else) used those coordinates when the POA left Reach on a "random" vector in accordance to the Cole Protocol (so not to lead the Covenant to Earth). Anyway, the Covenant was at Halo before the POA (they were looking for it and found it previous to the POA arriving). Hence, nothing was really luck or an accident when it came to the POA arriving at Halo.
