You've done a pretty good job summarizing all of the core questions regarding origins. For grins, I'll post the most commonly held (in my opinion, of course) theories for each below. DISCLAIMER: I know there are a billion theories..I just picked the most widely held and/or most logical ones. Also, none of the theories below are mine - I've plagarized every one of them.

: What is the connection between Humans and the Forerunner?

A lot of people believe than humans are the Forerunner. There are other theories that humans are related in other ways (part of the original covenant, Forerunner's servants, the MC's armor is somehow related or inspired by Forerunner technology, etc) but I won't cover them here. The main evidence for humans as Forerunner...
- 343GS talks to you like you are in the know, like a familiar. People tend to believe that this means he thinks you should know what he is talking about because you are a Forerunner.
- 343GS only chooses humans as Reclaimers,the MC and Mobuto (from the books).
- All of the specific things (many you have listed below) 343GS says to MC during the game further the case that he at least believes you are a Forerunner. Specifically, many believe that when he says "I'm glad to see some have survived to reproduce", he means Forerunner, as in I'm glad to see that you exist, meaning the Forerunner have survived to reproduce.
- In the books, time and again a strange familiarity to Forerunner controls and technology is described by the Spartans. Some take this to mean that we have instinctual memory (in our DNA?) that remembers what we used to know.
- In the books, blood from a Spartan's hand activates the Forerunners funky control room on Reach. Since DNA scanning for access is so widely used by the other races (Covenant and humans), many interpret this to mean that the DNA scan of the Forerunner technology recognized a Forerunner, and activates.
- There's some other stuff, but I think that's the most of it.

Again, many people take this to mean that 343GS was hanging out with the Forerunner 100,000 years ago when the Halo was fired. Many believe that 343GS is slightly confused (or completely stark-raving mad) and speaks to you like you were the same Forerunner he had a conversation with so long ago.

For both questions above, a lot of the theory on this is that the Forerunner had mad skills and technology - if the MCs suit is only class 2, what a class 12 suit would be like? That's right, it would be super-fly TNT. Your second question is great. Might it be possible that the MC'd go back in time to actually watch events from 100,000 years ago unfold? That would be most excellent, and not anything (I don't think) anyone has mentioned.

There are a million theories on this, but I'll use the one I think is the best. Covenant stumbled across humans and observed for a period of time. The Prophets (all-powerful religous leaders of Covenant) realize that the humans are the Forerunner. Because the Prophets' power base (i.e control of all other Covenant races) relies entirely on the worship and glorification of the Forerunner - which only they have "access" to or interpret for all the rest - the Prophets decide the truth can never come out and that humans (or Forerunner) must be destroyed. How would they explain that their diety is really this race of hairless monkeys with only mediocre technology? This raises its own thread of questions...how did we (the Forerunner) forget everything? What exactly happened 100,000 years ago?

Most believe they were not created - that they were a naturally occurring phenomenon (scourge?). This one is up for grabs though...I've heard some good theories that they were resultant from the Forerunners attempts to enhance their own DNA, so maybe they were created.

Assume for the following that they are natural. Some belive the rings were set up as research facilities to study the Flood because the Flood was destroying all in the galaxy. Think of the CDC and how they keep samples of the worst stuff in their labs to try to study and find cures. There is a lot of circumstantial evidence that the rings were fired approximately 100,000 years ago to stop a Flood outbreak, but no one knows for certain.

This is another one in which good theories abound, but I'll put one forth that I believe makes sense....the Covenant were there to get the Index, which is the all-access key to Halo 04. From the books it is implied that the Covenant (at least the higher ups) knew about the dangers of the Flood. A prophet had an Elite beheaded because he did not successfully avoid releasing the Flood on Halo. In the books, very little is said directly, but it seems that they have experience with the Flood.

: clear that the Flood escaped once before and Halo was fired to kill their

Some believe that when the Halos are fired, they fire "outward", and do not kill what is on them. It could also be possible that the last Forerunner set up all the hibernation samples, turned out the lights, and launched the Apocolypse, leaving the Flood behind for study by future races. The Flood on Halo was most likely able to go into hibernation for the 100,000 years because of the facilities. The books mention this hibernation pretty specifically. Some have asked why the Flood all across the galaxy (from the previous outbreak) simply didn't go into hibernation as well when the Halos were fired (they'd be all over the place now, wouldn't they?), and I think the best answer to that is that Flood can hibernate for really long periods of time, but not 100,000 years. Something in Halo's facilities allowed them to persevere.
